  1. The nature of the problem associated with physician awareness and education and physician involvement in quality assurance processes. Concentrate your efforts on how physicians are educated. Or aren’t they educated on this process?
  2. The steps taken to increase provider awareness and active involvement in the quality assurance process.
  3. What works and what does not.
  4. Your recommendations for further or continued improvements.
